In 2008 Fondazione Sanità e Ricerca integrated the Day Centre with the Alzheimer’s Home Care service in order to attend to patients and their carer throughout the course of the disease.
Home Care services are provided to sixty patients with mild to moderate dementia and behavioural disorders diagnosed by a Centre for Cognitive Disorders and Dementia, who live in the XI and XII districts within the GRA orbital motorway of Rome.
Home care is provided from Mondays to Fridays from 9.00 a.m. to 6.00 p.m. and Saturdays from 9.00 a.m. to 1.00 p.m.
As well as cognitive stimulation activities recognised in scientific literature - Reality Orientation Therapy, Memory Training, Occupational Therapy and Motor Rehabilitation, the centre uses specific experimental approaches for patients in the advanced stage of the disease, such as pet therapy.
Members of the family may attend educational meetings in order to understand dementia, its clinical expression and management of the most complex aspects, including behavioural disorders, nutrition, lifting and moving patients.
The home care service is scheduled weekly according to the needs of each Alzheimer’s patient and their carer, who may also rely on a stable team.
Members of staff use real time clinical data sharing software.
The Alzheimer’s Home Care service also offers advice on changes to be made in the home to:
- ensure safety;
- improve orientation;
- maintain cognitive skills;
- manage challenging behaviour.
Since the home care concept includes outdoor activities and the patient’s relationship with the community, the Alzheimer’s Home Care service provides group activities in the Foundation’s Alzheimer’s Care Centre as well as community integration events in association with various types of institutions, organisations and associations.
