The home care service is offered to patients with incurable chronic degenerative diseases in need of specialist palliative care that may be provided in their home.  The facility’s team assess the most suitable setting (hospice – home) on the basis of the information contained in the admission application form and the patient’s true living conditions.




The facility’s team determines an Individual Care Plan on the basis of a comprehensive and multidimensional needs assessment.  A comprehensive and multidimensional assessment and measurement of pain, is a specialist examination used in palliative care to assess patients clinical, medical, psychological and social status and needs.

Home care is provided by a multidisciplinary team according to the agreed Individual Care Plan and ensures that a doctor is available around the clock, 365 days a year.  A logistics service ensures that medications and medical aids are delivered directly to the patients’ homes.
